Checklist of Materials to be Submitted
Five copies are required; please return this form with your materials
- Personal Data Form*
- Personal History Sketch *
- Description of MCPCCE Specialization and Education to Include Career Goals *
- Supervisory Evaluations (ACPE, AAPC, etc...)
- Students Self Evaluations (ACPE, AAPC, etc...)
- Statement on Theology of Pastoral Care *
- Two Pastoral Relationships (verbatim, etc...)
- Letter From Your Bishop *
- Signed Statement Attesting Your Review and Knowledge of Synod's Policy on Sexual Misconduct
- Sermon Manuscript, Notes or Outline
- A Statement Detailing Plans for Obtaining Professional Certification
* denotes for guidance and consultation
Guidance and Consultation without a recommendation for MCPCCE Endorsement may be requested. In such cases, the committee offers their recommendations to the applicant regarding further personal/professional growth and development. The materials that need to be submitted are those which have asterisks. Other materials may be submitted in consultation between the candidate and endorsement committee convener.
