Five copies are required; please return this form with your materials

  • Personal History Sketch 
  • Statement of Vocational Goals 
  • Description of Clinical and Academic Education
  • Supervisory Evaluations (ACPE, AAPC, etc...)
  • Self Evaluations (CPE, AACPE, AAPC, etc...)
  • Statement on Theology of Pastoral Care 
  • Two Pastoral Relationships (verbatim, etc...)
  • Letter From Your Bishop 
  • Signed Statement Attesting Your Review and Knowledge of Synod's Policy on Sexual Misconduct
  • Sermon Manuscript, Notes or Outline
  • Plans for Obtaining Certification
