- Description of Clinical and Academic Education
- Supervisory Evaluations (ACPE, AAPC, etc...)
- Self Evaluations (CPE, AACPE, AAPC, etc...)
- Statement on Theology of Pastoral Care
- Two Pastoral Relationships (verbatim, etc...)
- Letter From Your Bishop
- Signed Statement Attesting Your Review and Knowledge of Synod's Policy on Sexual Misconduct
- Sermon Manuscript, Notes or Outline
- Plans for Obtaining Certification
