EVALUATIONS
Applicants funded in 2011 must complete and return the appropriate evaluation (see below) that corresponds with the same category in which the application was submitted.

You must have Formatta Filler - Full MB (3.17) installed on your PC computer in order to download and complete the evaluation. The Formatta Filler program is available for free online for a PC or Macintosh.

The evaluation forms have been designed so that applicants may type directly onto the form, save, and print. In some instances, the cursor may not appear right away when you click on a field in the form. If this happens, begin typing and the data will appear in the appropriate location.
